Wii U Mario Kart 8 Bundle no survey for Mario Kart 8

HI can someone answer this for me or direct me to who I can speak too about this. I purchased the Wii U Mario Kart bundle   I recieved my free game so nintendo club recognized that I have Mario Kart 8 .   I never recieved a Survey for Mario Kart  8 for the 70 coins.  I recieved one for the Wii U for the 160.   The Game and system did not come with a pin # for Nintendo club for Mario Kart 8 .   I am trying to figure out how I can get my survey for Mario Kart 8.
